Piercing MINOR CONSENT forms and RELEASE FORMS are only available in shop and must be completed the day of the service. Piercing minumum age is 10 years with legal guardian consent.

Here is a copy of our TATTOO RELEASE FORM. This is required prior to your tattoo. Feel free to print one, sign and bring it to your appointment. We also have copies available in the shop for you to complete when you arrive.

Here is a copy of the Official State of Florida MINOR CONSENT FORM. This is required prior to any tattoo performed on a minor, 16 or 17 years of age at the time of the tattoo. This form must be notarized before the tattoo can be started. We also have copies available in the shop for you to complete, take to get notarized, then return. A copy of the minor's birth certificate showing the legal guardian's name is needed also.
