TATTOO RELEASE FORM

This is required prior to your tattoo. Feel free to print one, sign and bring it to your appointment. We also have copies available in the shop for you to complete when you arrive. You will also need your physical ID with you at the time of your appointment.

TATTOO MINOR CONSENT FORM

This is the Official State of Florida TATTOO MINOR CONSENT FORM. This is required prior to any tattoo performed on a minor, 16 or 17 years of age at the time of the tattoo. This form must be notarized before the tattoo can be started. We also have copies available in the shop for you to complete, take to get notarized, then return. We require the minor's birth certificate, the minor's ID, and the legal parent who is listed on the birth certificate must be present with ID at the time of the tattoo.

PIERCING COSENT FORMS and MINOR PIERCING CONSENT FORMS are only available in shop and must be completed the day of the service. Piercing minumum age is 10 years with legal guardian consent. We require the minor's birth certificate, the minor's ID, and the legal parent who is listed on the birth certificate must be present with ID at the time of the piercing.
